JOB TITLE:           Community Transition Specialist (CTS)                   EFFECTIVE DATE:   8/22/2011

DEPARTMENT:  Independent Living                                                               REPORTS TO:  IL Program Manager

STATUS:  Full-time; 40 hour/week

Position summary:

The Independent Living specialist is responsible for coordinating the Community Transition program efficiently and effectively within the guidelines set forth by the Department of Health Care Policy & Financing to include successfully transitioning consumers from nursing home environments to independent living.

Minimum Qualifications:

Baccalaureate Degree preferred or equivalent experience

Experience and a desire to work with people with disabilities

The ability to arrange transportation and travel independently including the ability to travel freely throughout El Paso, Teller, Kiowa, Kit Carson, Park, Lincoln, Cheyenne and Elbert counties to provide IL services

Proficient with computer, including, Word, Excel, Power Point, Publisher, and Outlook

Knowledgeable or ability to become knowledgeable in social security benefits, Medicaid, Medicare, housing, and other services for people in CTS.

Ability to work as a team member

Essential Functions/Areas of Accountability:

Ensure the philosophy, mission and goals of the independent living program are maintained.

Have a working knowledge of the Americans with Disabilities Act, the Rehabilitation Act, and Fair Housing

Identify nursing home residents for CTS eligibility

Determine if residents were referred through MDS Section Q  or other means

Initiate assessments and intake per The Independence Center and State requirements

Work with Social Service agencies and case managers following set procedures

Actively promote and market the program as well as maintain a list of eligible consumers

Conduct nursing facility in-service and education concerning CTS program

Make referrals  as needed for independent living skill services

Data entry and charting, maintaining updated consumer records

Initial assessments and intake per The Independence Center (THE IC) and State requirements

Facilitate team effort with other agencies, family, and friends of the consumer

Coordinate transition— ensure all benefits and social services are utilized (housing voucher, food stamps, home health services, lifelines)

Assist consumer with initial shopping for food,  household items and furniture

Lifting and carrying purchased items, arranging for delivery as needed

Coordinate follow up visits as required by THE IC and/or HCPF

Establish and maintain cooperative working relationships with coworkers and the public

Responsible for completing all paperwork and documentation such as Consumer Intake packet, Individual Living Plan and entering services into Services database to meet State regulatory requirements in a timely manner.

Be a self-starter who is productive and proactive as well as work in conjunction with current CTS Specialist to accomplish job duties.

Attendance at weekly staff meeting, engage in and explore opportunities for training related to the position.

Set clear expectations, responsibilities and goals with consumers.

Maintain confidentiality as outlined in the employee handbook and work within the confines of HIPPA regulations.

Accepts responsibility for personal and professional development.

Performs other related duties and responsibilities as deemed necessary.

Skills and Abilities:

Compassion for people wanting to get out of nursing home

Personal experience with a disability and knowledge of the Independent living movement is desirable

Excellent Interpersonal Skills

Excellent Communication Skills, Including Public Speaking

Ability to write routine reports, assessments and related correspondence.

The capacity to empower individuals with disabilities to advocate for themselves

Physical/Environmental Requirements:

Ability to lift, push, pull, drag, guide up to 35 lbs.

Ability to lift, carry, bend, stretch, climb, crawl, twist and maneuver without any restrictions.

Ability to sit or stand for extended periods of time
